Summary

Are you passionate about supporting individuals transitioning from prison back into the community?

The Role
Join our dedicated team as a Prison Leaver Recovery Practitioner, where you’ll play a vital role in improving continuity of care, reducing reoffending, and empowering people on their recovery journey.

Working within a hybrid model, you’ll be part of a flexible, multi-disciplinary team supporting men and women pre- and post-release. You’ll build strong relationships with prison substance misuse teams and community services to ensure seamless transitions and sustained engagement with drug and alcohol support.

The Service
Via Harrow is an established Community Integrated service that is well performing and highly functioning. The service consists of both clinical and psychosocial and is CQC registered to provide a range of clinical interventions including BBV & health interventions, OST, and community detox, as well as robust family and carers and service user involvement provision.

Location
Harrow, with travel to HMP Wormwood Scrubs, Bronzefield and Willesden Magistrates courts.
