Summary

To work towards the development of a new follow up MDT critical care clinic and development of a dietetic follow up service To work as part of the multi-disciplinary team leading and providing dietetic input to patients in specialty and to liaise with other disciplines and agencies as required in the planning and implementation of patient care To provide highly complex nutrition counselling to patients and/or their carers requiring post registration cognitive behavioural therapy and use of reassurance and motivational skills to overcome barriers to change particularly where there is non-compliance To use negotiating and persuasive skills to facilitate effective service delivery within the specialty team. To liaise with dietitians within the critical care team and ensure overall prioritisation of workload within the team supporting other members as appropriate. To maintain lines of communication within the Directorate To actively participate in ward meetings, ward rounds, case conferences as appropriate. To liaise with and work as an integral member of the multi-disciplinary team, building and maintaining effective working relationships. To lead on the design, implementation and evaluation of multi-disciplinary protocols and procedures aimed at improving nutritional status To use motivational and negotiation skills in the treatment of patients in your specialised area to gain consent and compliance with treatment To communicate and process complex and sensitive information to and from patients, carers and multi-disciplinary team. To attend the Critical Care Governance, Rehabilitation Service Development & Critical Care Rehabilitation Working Committee meetings Liaise with other agency service providers (including GPs, Clinics, Community / Outpatients Service, District Nurses, Social Services). Liaise with Dietitians from other Trusts and other Healthcare professionals concerning patient treatments and progress with dietary regimens, depending on the patients clinical condition and need.
