Summary
Are you a registered nurse who is passionate about diabetes, capable of working autonomously and interested in further developing your knowledge, skills and competencies towards Band 7 level? We’d love you to come and join us in the diabetes team!
We are looking for a highly experienced Band 5 or Band 6 nurse to join the multidisciplinary diabetes team to support outpatient diabetes services initially to be trained to lead the diabetic foot clinic.
Based in the Diabetes Education Centre, it is a permanent post and is 20 hours per week, to include Mondays, Wednesdays and Fridays for diabetes foot clinic cover.
Ensure evidence-based practice and the highest standard of personalised clinical nursing care is delivered to all patients with diabetes and their families attending CRH outpatient areas.
To work autonomously within the multi-disciplinary diabetes foot clinic team to ensure the provision of safe, effective and optimal diabetes care.
Provide specialist nursing care and advice to a specific patient group, mindful of cost-effective use of resources and contribute to the development of the speciality service.
Manage a defined caseload, carrying responsibility for the assessment of care needs, the development, implementation, and evaluation of programmes of care.
Exercise judgement and autonomous decision making daily in assessing wide ranging and complex patients’ problems, agreeing solutions, ensuring and promoting options to enable the delivery of optimum patient care, advocating for patients where appropriate and advising the multi-disciplinary team.
Provide expert advice on the care of people with complex diabetes or unusual regimens. Work within own competencies and scope of practice to adjust diabetes therapies as required.
Provide face to face and telephone support and information for both new and existing patients, their carers and health care professionals, including the teaching and self-management of insulin therapy and diabetes technologies.
