Glasgow City Health and Social Care Partnership (GCHSCP) is seeking to recruit an Assistant Project Leader within North West area of the city. Applicants are invited to apply for the post to support the delivery of the Community Support Team, which provides a Person-Centred Home-Based Support service for older people who have a diagnosis of dementia and associated health and physical support requirements.

The Assistant Project Leader will be required to screen referrals, complete needs assessment of older people and their carers and manage a caseload with high demand. The successsful candidate will provide direct line management to Support Workers and will be responsible for day-to-day supervision of service provision. The successful candidate will also deputise for the Project Leader when required.

The Assistant Project Leader will have a lead role in developing improved outcomes for older people with dementia through the provision of person-centred care in line with National Health and Social Care Standards. You will be expected to work on your own initiative as well as part of a team and work with HSCP staff, partner organisations and the third sector to ensure effective communication regarding service user and carer’s needs. You should have knowledge and awareness of dementia, the effects of ageing and disability on service users and their families, a flexible approach to service delivery, and the ability to communicate effectively.

The successful candidate will be required to work Monday to Friday, 35 hours per week on an agile basis and participate within the on-call provision for the Good Night Team Service over seven days.

Membership of the Protection of Vulnerable Groups (PVG) Scheme is essential on taking up appointment. Please note successful candidates for the above role will be required to pay for their PVG Membership/or Scheme Checks.

Registration to the Scottish Social Services Council (SSSC) is an essential requirement for this role. You must either be already registered or have applied within 3 months and be registered within 6 months from your start date.
