Summary
Perform specialist assessment of patients nursing needs, plan and implement and evaluate care using your clinical knowledge and skills. Coordinate and manage nurse led vascular clinics. Contribute towards the multi professional approach in the management of patients within the specialist area, especially in relation to ongoing care and discharge arrangements Establish and maintain effective communication with patients and carers and relatives and other members of the MDT. Support patients and their family / carers through their vascular journey. Recognise changes in patients condition that require intervention of others and refer on as appropriate. To work within the specialist nursing team to contribute to the service in partnership with lead clinicians, general managers, senior nurses, and matron to develop the service and to ensure high standards of patient care.
