Summary
Audit and data collection Daily collection and processing of physiological Critical Care patients data to comply with ICNARC protocols and audits. Inputting clinical data into an secure online platform of all admitted Critical Care patients, this requires high level attention to detail and robust data cleansing. Skilled use of clinical programs for collection and interpretation of patients data, like Careflow, ICE and Vitals Clinical. Validation of Critical Care data as per ICNARC coding system. This involves liaising with ICNARC senior nurses and consultants to ensure correct coding of diagnosis, alongside filling in missing data, completing hospital outcomes, etc. Create Monthly reports for the department, further departments within Whittington Trust and other external NHS bodies, for the purpose of governance, safety and QI work. Liaise with nursing and medical staff to ensure accuracy of clinical data entry Prepare regular clean dataset for export to ICNARC, monthly, quarterly and yearly. Liaise closely with external bodies like ICNARC and the North Central London Network and other hospitals with regards to data enquiries, analysis and reports which involves using clinical electronic systems. When dataset not complete retrospective cleaning of data via ICNARC platform Processing of Level 2 patients data cared outside the Critical Care unit into the Careflow in collaboration with the Trusts Finance Department. Collecting demographic and clinical data for patients outside the Critical Care unit with CC team involvement and manual processing into the ICNARC website. Implement system upgrades when instructed by the units clinical management system supplier. Attend regular meetings with ICNARC as part of CPD and maintenance of the high profile of the trust within that organization. Contribute to the on-going education of the multi-disciplinary team as to data collecting, entry and reporting with regard to Critical Care Audit. To liaise with other hospital staff and departments in the data management of patients transferred in and out across different hospitals. To be responsible for ensuring confidentiality and safekeeping of patient records. Adheres to Trust and Directorate policies, procedures, standards and protocols To maintain confidentiality at all times Minute taking and meeting coordination for Critical Care Department and Deteriorating Patient In conjunction with the Critical Care Team, coordinate regular unit meetings such as clinical governance and Mortality and Morbidity meetings and staff meetings Minuet take for Clinical Governance, M& M and Deteriorating patient improvement Group Coordination action tracker for all meetings. Admin support for all QI project across Critical Care Administration for Follow Up Clinic Work with follow up team to identify and enter spreadsheet potential patients for follow up clinic monthly When patients identified send letters to all patients once discharged from hospital Following invite letter, to telephone patients and discuss dates for clinic using spreadsheet Book patients into clinic via Medway On clinic days to welcome patients give out psychological questionnaires.
