Summary
Role Purpose
To protect vulnerable individuals, advocate for their welfare, and support them toward safety, independence, and well-being, through assessment, intervention, and collaboration.
Key Responsibilities
Conduct assessments to identify care needs for individuals, families, or groups and develop tailored support plans.
Provide emotional, practical, and therapeutic support, and ensure safeguarding measures where necessary.
Liaise with and coordinate services across health, education, housing, and other agencies in multi-disciplinary team meetings.
Maintain accurate records, produce case notes and reports for decision-makers or court proceedings if required.
Attend managerial or supervisory reviews, and engage in case discussions to continuously improve practice.
Complete statutory duties and uphold safeguarding protocols in line with Making Safeguarding Personal principles.
Qualifications & Registration
A recognized social work qualification (e.g., BA/BSc/Master's in Social Work, PGDipSW), or an equivalent approved by Social Work England.
Mandatory registration with Social Work England to practice legally.
Enhanced Disclosure and Barring Service (DBS) check clearance.
Full driving license and access to transport preferred, for travel across the local area.
