Summary

Coordination and Service Management Coordinate and actively promote Enhanced Access and additional extended services across all Gateshead Inner West PCN member practices and partner organisations. Serve as the primary liaison between service providers, clinicians, administrative teams, and external stakeholders to ensure seamless communication and efficient service delivery. Manage and optimise clinical and administrative service schedules, including creating, updating, and amending clinical rotas and appointment clinics on EMIS and other systems. Monitor appointment availability in real-time to maintain optimal access for patients, intervening where necessary to resolve scheduling conflicts or bottlenecks. Review appointment bookings regularly for clinical appropriateness, flagging any issues or trends to the relevant practices or clinicians for action. Actively gather and analyse patient feedback and complaints related to Enhanced Access services; assist in logging, coordinating investigation, and supporting resolution in line with practice and PCN policies. Support the implementation and rollout of new service developments, pilots, and improvement initiatives, liaising closely with clinical leads and management teams to ensure smooth adoption. Monitor service performance indicators and patient outcomes, producing detailed reports, data summaries, and insights to inform PCN decision-making and board discussions. Administrative and Operational Support Maintain accurate, up-to-date records and databases relating to service delivery, appointments, staffing rotas, and patient feedback, ensuring compliance with data protection standards. Act as a first point of contact for patient and public enquiries related to Enhanced Access services, providing timely and clear information or directing queries appropriately. Develop, review, and assist in implementing operational protocols, standard operating procedures, and workflows aimed at improving efficiency, patient experience, and service quality. Provide flexible support to the day-to-day operations of the PCN, including reception duties, data entry, communications, and other administrative tasks as needed. Assist the PCN management team with scheduling meetings, preparing agendas, taking minutes, and following up on action points related to Enhanced Access and wider PCN services. Facilitate communication and coordination between multiple member practices, ensuring consistency of service delivery and shared understanding of policies and changes. Additional Duties Support training and induction of new staff or temporary cover relating to Enhanced Access clinics and administrative processes. Monitor compliance with contractual requirements and service level agreements relating to Enhanced Access services, escalating any concerns to PCN leadership. Participate in PCN-wide projects and initiatives as required, contributing expertise on patient access and operational workflows.
