Summary
The post holder will work across the primary, community, secondary, and social care interface to deliver holistic, patient-centered care for frail and elderly patients within the community and primary care network setting. Actively assess, plan, implement, and evaluate care across the full patient pathway in partnership with multidisciplinary teams (MDT), patients, and carers. Utilise clinical expertise in frailty and community nursing to support patients independence and wellbeing. Work within NHS structures and adhere to the Nursing and Midwifery Council code of conduct to maintain professional standards. Contribute to service development through clinical governance activities, audit, and quality improvement initiatives. Support and mentor junior colleagues, student nurses, and healthcare assistants to develop clinical skills and knowledge relevant to community and frailty care. Provide leadership within the team and deputize for senior staff when required. Must hold a full, clean driving licence and have access to a car for daily use to enable travel across the PCN area. Position is part-time, 3 days per week. CORE ROLE & RESPONSIBILITIES Professional Leadership / Management Work collaboratively with MDT to ensure coordinated care for frail, elderly patients, supporting their health and independence. Mentor and support junior staff, student nurses, and healthcare assistants through supervision, assessment, and induction processes. Act as a clinical role model, promoting best practice and professionalism. Identify and address challenges affecting patient care delivery, empowering team members to make decisions. Lead care coordination or team activities in the absence of senior staff. Ensure patients and carers views inform care planning and service improvement. Lead on specific clinical initiatives or projects related to frailty and community care. Clinical Practice Maintain and develop clinical competence in frailty assessment tools and relevant community nursing skills. Provide expert nursing care to a defined caseload of frail elderly patients, managing complex needs across settings. Deliver health promotion and education tailored to the elderly and their carers, supporting self-management. Provide clinical advice and support to colleagues across the PCN and community. Ensure all care is documented accurately in line with local and national standards. Coordinate nurse-led interventions, including falls prevention, medication reviews, and advance care planning. Collaborate in developing and delivering integrated care pathways for frailty management. Promote dignity, respect, and individualised care in all patient interactions. Clinical Governance Participate actively in appraisal and personal development planning. Comply with mandatory training, clinical policies, and NMC standards. Engage in audit, research, and quality improvement activities related to frailty and community nursing. Report incidents and participate in investigations as required, supporting risk management and patient safety. Assist in delivering competency-based training programs for the team. Contribute to service evaluation and development to enhance care for frail elderly patients. Nurse Responsibilities Maintain active registration with the NMC and adhere to professional standards. Keep clinical skills and knowledge updated, maintaining a professional portfolio. Comply with Trust and PCN policies, including confidentiality and information governance.
