Summary
JOB SUMMARY: To work as a member of the multidisciplinary team which provides a range of specialist services to clients requiring assessments for DRR/ATR disposals and to ensure the appropriateness of the Order. To provide care to DRR/ ATR service users presenting with substance use issues, including alcohol to deliver evidence-based interventions such as: harm minimisation approaches, motivational interviewing, relapse prevention, care planning individual intervention, health education and detoxification programmes, mindfulness, counselling and group work in 1-1 and group work settings. To provide progress reports for the offender manager and Courts. This may also include supporting/delivering the structured day programme. Contribute to sentence plans & breach proceedings where appropriate. Build effective partnership working with the prison and courts to ensure a timely response to requests, including timely appointments for prison releases & Through the Gate arrangements/escorts. To participate in multi-disciplinary meetings, both on-site and in the community relating to client management. Engage in the Safeguarding, MAPPA, MARAC & IDVSA process and information sharing protocols. Develop and enhance strong partnership working and liaison as appropriate to the service and refer to & engage agencies as appropriate (inclusive, but not exhaustive; prison, police, NPS, CRCs, Liaison & Diversion Teams, Mental Health Services, GPs, Housing, ETE, Social Services) To make continuous assessments of the psychological, emotional, social and rehabilitative needs of clients, taking into account the needs of family, local community and developing care plans and evaluation of these plans
