Summary
The CHC Appeals Team is an integral part of the wider NHS BSOL All Age Continuing Care Team (AACC). It has strong links with a wide range community, acute and tertiary services and third sector providers. We also work closely with NHS England and the regional Independent Review Panels. This position requires a proactive, collaborative working style to promote effective relationships within the multi-disciplinary team and with our community colleagues in health and social care is essential. The post holder will play a key role in supporting the All Age Continuing Healthcare Team in delivering effective, responsive services across the BSOL region.; working within the remit of the National Framework for Continuing Healthcare and PUPoC Guidance. The post holder will have both experience and a high level of confidence and understanding/experience of working to the Best Practice Guidance of the NHSContinuing Health Care Framework (2022) The post holder will produce comprehensive reports, including Care Needs Portrayal documents identifying a patients journey throughout the period of review for both Appeals and PUPoC. have a high level of communication skills across both written and verbal media, to be able to leadand Chair Local Resolution Meetings with patients/individual's and their representatives deliver an effective and competent level of service and consistently deliver a client-focused service. work autonomously to individual targets and team key performance goals to progress andmanage a caseload of patients as directed and support colleagues within the appeals team as required to enable effective case coordination and respond to queries from all interested parties.
