About the Team

This is an exciting opportunity to join our Specialist Adult Social Care team in Keynsham as a Transitions Social Worker. You’ll play a vital role in supporting young people aged 16 to 25 as they move into adulthood, helping them navigate complex needs and access the right support at the right time.

You’ll be part of a warm, collaborative, and forward-thinking team, working alongside a wide network of professionals including CAMHS, SEND, Children’s Services, AWP Mental Health Teams, and more. It’s a chance to make a real difference in the lives of young people and their families.

About the Role

As a Transitions Social Worker, you’ll manage a diverse caseload and work with a high degree of autonomy, supported by an experienced Team Manager. This is a role where your expertise and initiative will be valued—and where your impact will be visible.

Your responsibilities will include:

Leading adult care assessments and reviews under the Care Act 2014
Designing personalised care and support plans
Conducting Mental Capacity Assessments and Best Interests Decisions
Managing risk through thoughtful, person-centred strategies
Completing Section 117 Aftercare reviews and Continuing Health Care Checklists
About You

We’re looking for someone who is passionate about empowering young people and committed to high-quality social work practice.

You’ll need:

A recognised Social Work qualification (Degree, Diploma, CQSW or equivalent)
Registration with Social Work England
Experience supporting individuals with autism, mental health conditions, learning disabilities, or physical disabilities
A Full UK Driving Licence and access to a vehicle
