We are looking for an enthusiastic and motivated Physical Health Support Worker to join our busy and friendly team .
The Later Life Team supports Adults over the age of 65 years who have secondary mental health needs, both organic and functional as well as carrying out Memory Assessments.
We cover the Central Manchester area and support people living in their own homes, sheltered/Extra Care Housing or in care homes.
We are looking for someone who is committed to the health needs of Older Adults and whilst this post does not require professional registration, experience of mental health is essential.
The post holder's professional development will be supported by the service through regular supervision and training opportunities
Please note - This role is not eligible for sponsorship unless you meet criteria by temporary exemption from current changes to immigration rules put in force on 22/07/2025. Separate provisions are applied to workers who have been sponsored and held continuously Skilled Worker visa since prior to 04/04/2024.
The post holder will complete physiological assessments of our service users.
This includes taking and recording physical observations such as blood pressure, pulse, and weight as well as Venepuncture. Training will be available for this for the right candidate
Supporting the Memory Treatment pathway by undertaking relevant observations
Assessing service users holistically and completing Physical Health Assessments. These visits will take place in service users’ homes
Be an active and contributing member of the Community Mental Health Team and feedback to MDT as needed
The postholder will provide information on health promotion which may include smoking cessation, alcohol use, dietary needs appropriate to our client group
