Summary
OVERALL OBJECTIVES
1. To provide support to people with a physical disability and or learning disability as well as mental health conditions to enable them to maintain their tenancies and to achieve their aspirations for independent living.
2. This job description is generic and applies to all support workers. The duties will be determined by the individual needs of the users being supported. The context in which support is been provided may vary according to the user’s support plan.
3. Depending on location the support provided may vary from the supported living services to NDRC Day Centre and in the community.
KEY TASKS
1. Identify users’ individual support needs and, through consultation and
agreement with the user (and, where appropriate, their Care
Manager), devise and implement suitable support plans to ensure
that identified support is given to the highest standard.
2. Promoting safety and providing emotional support.
3. Assisting in or providing total personal care to patients.
4. Observing and reporting and substantial or unusual findings in
Service users’ behaviour or physical condition.
5. Assisting with meal preparation.
6. Performing routine household chores.
7. Listening to the concerns of Service user’s and reporting any
concerns to senior management.
8. Promoting safety and providing emotional support when needed.
9. Administering safely and correctly a Service user’s medication.
