Summary

To lead clinical pharmacy services for patients treated within the Care Group in accordance with the priorities set by the post holder, Deputy Chief Pharmacist, Clinical Services, General Manager and Clinical Director(s) in order to ensure safe, clinically effective and cost efficient use of drugs in these patients To operationally lead and be accountable for the Pharmacy service delivered at the Caldecot Pharmacy, ensuring that the service is delivered in line within legal and quality standards and meets the needs of service users To lead locally on the delivery of clinical trials delivered to service users of the Caldecot Pharmacy, working in collaboration with the Associate Chief Pharmacist, Clinical Trials to ensure that requirements are met To lead on Pharmacy and medicines use for the Haven and Sexual Health Services. To develop clinical pharmacy services for patients in the Care Group in accordance with objectives set in order to advance the safe, clinically effective and cost efficient use of drugs in these patients. To identify a vision for the delivery of pharmaceutical services to the Care Group. To deliver this vision within the boundaries of national and local priorities. To deliver the clinical governance agenda in relation to drug use in the Care Group and be an active member of the Care Groups clinical governance committee. To motivate and inspire others (both within pharmacy and the Care Group). To proactively develop clinical pharmacy services in line with local and national objectives. To demonstrate innovation. To extend the boundaries of the profession. To ensure that issues relating to pharmaceutical care are appropriately represented within the Care Group. To work collaboratively with senior pharmacy and medical staff, the multidisciplinary team and management and to participate in the development of pharmacy services in accordance with the agreed objectives. Responsible for policy implementation and service development of the specialist pharmacy services within the Care Group. To manage and develop the provision of clinical pharmacy services to all wards, units and departments in the Care Group in accordance with the priorities set by the post holder, Deputy Chief Pharmacist, Clinical Services, General Manager and Clinical Director. To work with clinicians, nurses and other Trust staff to develop, implement and monitor policies, guidelines, protocols and procedures at Trust and regional level, as required. To propose and comment on policy and service changes and plans within the clinical pharmacy service. To contribute to, and participate in, pharmacy service developments, and co-ordinate and implement them within the Care Group, in line with departmental and Trust policy. To ensure services provided are in line with national agendas, policies, guidelines and frameworks. To actively participate in and contribute to the development, promotion and local implementation of local and regional prescribing and clinical services guidance. To establish processes for the setting of standards of care To work with the Deputy Chief Pharmacists to strategically develop clinical or operational pharmacy services across the Trust To lead on specific projects or initiatives across the Pharmacy Service To be a member of and actively participate in Trust-wide committees relevant to role. To develop clinical pathways in the specialty. To work with the Principal Pharmacist, Clinical Services, Formulary Lead and Consultants to support applications for drugs to be added to the Joint Formulary To ensure the compilation and maintenance of a comprehensive resource file for use by on-call staff and other pharmacists To be responsible for the provision of a highly specialised pharmacy service (clinical, dispensing and supply) to patients within the Care Group. To provide highly specialised evidence-based clinical advice and information (doses, interaction, adverse effects, drug availability, complex dose calculations) to nursing and medical staff (within the Trust and externally) on the management of individual patients, in accordance with legislation. To act as a pharmacist independent prescriber to optimise the care of patients. To participate in consultant ward rounds.
