Summary

About us: The Intake and Assessment team is a dynamic and exciting team with a varied and interesting workload. Our function is to:

Provide a timely response to Safeguarding concerns, undertake sec 126 enquiries and liaise with partners to develop safe care & support plans, respond to people experiencing crises through assessment, support and family liaison and the commissioning emergency placements/care packages when necessary. Complete outcome focused assessments ensuring the full participation of service users and/or their carers. Respond to urgent requests for equipment/manual handling concerns
Promoting people’s wellbeing through the provision of Information, Advice and Assistance via our Wellbeing Matters Service. Review or assess and people who are known to Adult Services but whose needs have changed and may require a new assessment. Develop Care & Support Plans in partnership with citizens to promote their wellbeing and social role within their community.

About the role:

Pay Details: Grade 6 SCP 14 – 19 (28,624-£31,067)
Hours of Work / Working Pattern: 37 hours Monday to Friday
Main Place of Work: C1V
Description: To support Adults Services in managing the initial contacts from the public, and processing enquiries through effective signposting, and initial assessments. To undertake UA reviews, provide information and support to social workers with assessments and reviews and monitoring of user and carers needs.


About you / You will need:
• Experience of working in a Team
• Some knowledge/ understanding of ageing/illness/disability and its effects of individuals/families and their Carers
• Good standard of written and verbal communication skills.
• Ability to organise and prioritise tasks.
• Creative and imaginative approach to problem solving.
• Ability to work under pressure
