Responsibilities
Conduct comprehensive assessments of clients' needs and strengths.
Develop and implement individualized care plans in collaboration with clients and their families.
Provide therapeutic support and counseling to clients facing social, emotional, or financial challenges.
Facilitate access to community resources and services, including housing, education, and mental health support.
Collaborate with other professionals and agencies to ensure a coordinated approach to client care.
Monitor clients' progress and adjust care plans as necessary to achieve desired outcomes.
Document all interactions, assessments, and interventions in accordance with legal and organizational policies.,
Participate in team meetings, case conferences, and training sessions to enhance service delivery and professional development.
Requirements
A degree in Social Work or a related field, with appropriate social care qualifications.
Proven experience as a social worker, preferably within a community or social care setting.
Strong understanding of social policy, welfare systems, and community resources.
Excellent communication and interpersonal skills, with the ability to build rapport and trust with diverse clients.
Demonstrated ability to work collaboratively within multidisciplinary teams.
Strong organizational skills and ability to manage multiple case loads effectively.
Commitment to ongoing professional development and adherence to ethical standards in social work practice.
