Summary
The Community Specialist Palliative Care Home Team (SPCHT) at Arthur Rank Hospice Charity supports patients with life limiting conditions across Cambridgeshire. Supported by the wider multi-disciplinary team at the Hospice, the SPCHT ensures that our care isholistic and tailoredto each individuals personal situation. Our Clinical Nurse Specialists deliver expert care and support to specialist palliative and end of life patients and their families/carers. Working closely with other care providers our team will help manage symptoms, support with medicine management and help access other hospice services, such as complementary therapy. The service runs 7 days a week and colleagues will join us and work on a rota basis to support this. Main duties will include: To holistically assess, plan, implement and review therapeutic interventions of patients with complex needs Organisation and management of individual caseloads, prioritising and responding to clinical emergencies as required Plan and evaluate programmes of care for patients and carers, undertaking assessments and analysis of physical, psychological and social needs of the patients. Regular communication with other care providers, such as GPs and District Nurses Working as part of the community team to effectively communicate with members of the MDT Working as part of the senior nursing team, providing specialist nursing advice, support and education to other health care professionals and volunteers and deputise for the Matron in their absence as required. To be successful in this role you will be a Registered General Nurse with a Post-registration qualification in cancer/palliative care, or long-term conditions (or a willingness to work toward this.) Its essential to have experience delivering specialist palliative care in a multi professional environment, along with experience of teaching. Due to the nature of the role its a requirement to hold a valid driving license and have access to a vehicle for work purposes. This post is subject to an Enhanced DBS Check with Child and Adult Barred lists. Starting salary of £47,810, with opportunities to progress to £54,710. Please note that the salary quoted is based on full time hours and will be pro-ratad for part time posts.
