University Hospitals Dorset (UHD) is seeking a band 6 Specialist Speech and Language Therapist to join our forward-thinking Early Supported Discharge (ESD) Team — a leadingservice that transforms patient recovery by delivering specialist care at home.

To provide assessment and intervention for people with communication disorders and dysphagia.


Assess,planand deliver individualised programme of intervention evaluating the effectiveness for patients presenting with a range of conditions. Giving advice to patients and their carers when required.


Provide a high standard of interventions to patients with diverse presentations referred to the service.


Plan and carry out assessments of patients’ needs,taking into accountinformation provided by carers and multi-disciplinary teams and develop and deliver an individual treatment programme.


To hold responsibility for own caseload working under the supervision of a Clinical Specialist Speech and Language Therapist.


Contribute to the discussions on service and policy development and implement in theworkplace as directed.


Identifythe current and potential future needs of the patient andbe responsible forinitiatingappropriate referralsto a variety of internal and external agencies.


Toassistthe ESD Team Lead with data collection, SSNAP, audits and reports so that the service can bemonitoredand developed.


Toassistwith neurological education to therapy assistants, peers, students on placement, the widerteamand external agencies.

Cross site working as required.
