Summary
Working as part of the developing West Midlands Learning Disability and Autism (WMLDA) Community Forensic Service, the post holder will: Use expert clinical knowledge to support Integrated Care Systems (ICSs) and commissioners in progressing discharges to appropriate community placements or care packages. Work proactively with inpatient providers to identify individuals approaching the end of their treatment pathway and prepare for timely discharge. Support the implementation of the national 12-point discharge planning framework and ensure legal and procedural frameworks are applied to prevent unnecessary delays. Monitor discharge readiness and report on delays, providing data and insight to key forums and stakeholders. Lead and coordinate multi-agency escalation meetings to address and resolve barriers to discharge. Work in close partnership with ICS colleagues, community forensic teams, and Reach Out Case Managers to reduce the number and length of delayed discharges. Provide expert clinical advice and consultation on best practice in care and discharge planning for people with learning disabilities and/or autism, promoting personalised, needs-led commissioning.
