Glasgow is a thriving and vibrant City and excellent opportunities exist to join us and contribute towards helping our neighbourhoods flourish. The Glasgow City Health & Social Care Partnership (GCHSCP) is on a journey to radically change the way in which Social Work delivers support and services.

We are looking for qualified Social Workers who are committed to making a real difference to the lives of older people and adults.

Within Older People & Primary Care there is a significant transformation programme to ensure that we support adults with multiple and complex needs and older people with increasing levels of frailty to live as independently as possible. Older peoples services are fully integrated across the city in order ensure that we support older people and their families / carers as effectively as possible.

We have a large range of services that require to work effectively together in order to ensure that the most vulnerable within our City get the right service in the right place at the right time. We work in full partnership across both adult and older people’s services with our health colleagues and a broad range of partners and we have a key priority in relation to shifting the balance of care across both service areas and promoting more effective early intervention and preventative supports.

We are seeking energetic, talented, resilient and committed Social Workers to secure these outcomes. You will have sound knowledge of assessment and care management; have practiced extensively under the relevant legislation; and work in a person centred manner which will be fully embedded in your practice. You will be confident in managing risk and looking for innovative ways to promote independence and link people into the local community.
