Summary
To link in with existing mental health provision for young people and their families (CAMHS, TAMHS), as well as linking with other local organisations/agencies who work with young people e.g. colleges To attend CAMHS transition panel meetings to review support for a young person and relevant pathway of support. To attend CPA meetings via CAMHS to support To arrange 121 wellbeing appointments with people assigned for wellbeing support. To provide monthly support for young people for up to 12 months with the goal of completing a wellbeing plan (with follow up review appointments) and ensuring the person feels supported to move forwards with their mental health journey. To support individuals to develop wellbeing plans based on the 5 Ways to Wellbeing. To provide 121 wellbeing support for young people self-referring into the wellbeing service. To promote resilience by supporting individuals to develop and maintain their own recovery plans. To provide Step Down appointments (appointments can be outreach in the community) for up to 12 months per young person. To provide wellbeing step down support for adults being discharged from secondary mental health services as and when needed. To support other areas of work and undertake other duties at Swindon & Gloucestershire Mind when needed To lead on the Young Person group promoting the session externally and encouraging young people involved to be involved with the setup of the session plan. To encourage Young Persons to become Experts By Lived Experience to help shape the services we provide to meet the needs of young people. To deliver the 5 Ways to Wellbeing Programme in schools working with young people and using a variety of training and facilitation methods tailored to the needs of different groups. To work closely with local authority care leavers team to ensure a joined-up approach to supporting young people's mental health. To effectively record and evidence outcomes achieved by young people. To provide social media content on CYP. To meet a number of wellbeing plans set as monthly targets by manager. To meet a number of reviews set as monthly targets by manager. To use Views database system or an alternative data software (as appropriate) to capture, track and monitor performance.
