Summary
To provide advice and information under the Care Management/Occupational Therapy Process for Adults and Older People which could lead to service provision or referral on to other departments, agencies and/or organisations. To visit adults and older people in their own homes or other community settings in order to assess, implement, monitor and review service provision either functional or care). To ensure that all work complies with Council policies, procedures and financial framework; and that statutory obligations are met. To be responsible to the Line Manager for: The quality, and prioritisation of own work and for alerting the Line Manager to any caseload/workload management issues. The adoption of own good practice and performance standards, adherence to Directorate policies, procedures, statutory responsibilities and agreed performance targets. Main duties of the post 1.To coordinate services within eligibility criteria whilst adopting a flexible approach to support service users and carers. 2.To provide intensive screening and complete assessments of multiple needs often across a range of disciplines (e.g. rehabilitation, benefits, mental health and equipment), using essential skills and detailed knowledge of current policies, procedures and government legislation regarding service delivery including the prevention and personalisation agenda. 3.To use essential skills to prioritise, calculate risk factors and respond to situations in an emergency under close supervision. 4.Under minimal supervision ensure referrals to other teams/services encompasses information relevant to the appropriate service area that may have a significant impact on the service user and/or carer. 5.To assess for, recommend and make arrangements for the delivery and collection of simple equipment as required. 6.Develop and implement care plans for service users/carers with a range of needs using either care management/occupational therapy models. 7.To have an awareness of all issues in relation to the Safeguarding of Adults and to identify any cases or issues under a level of supervision. 8.To ensure appropriate recording in connection with work provided to service users, carers and other departments in line with Council procedures and appropriate to service requirements. 9.To respond to, and collate information relating to enquiries, complaints and service provision issues as requested by the Line Manager. 10.To participate in ensuring that performance indicators are met. 11.To respond appropriately to unscheduled events as directed by the line manager. 12.To ensure the safe management and security of equipment provided by the Council including personal responsibility for sample aids during transit and testing. This may also include the fitting, demonstration of, review of or advice relating to assistive equipment and minor adaptations. 13.To undertake assessments under supervision, within the Mental Capacity Act e.g. best interests. 14.To undertake all duties with due regard to the provisions of Health and Safety regulations and legislation, Customer Care Policies, and new technology agreements. 15.To ensure that at all times, work undertaken actively promotes the Councils Equal Opportunities Policy in relation to service and personal practice. 16.To ensure that all individuals are treated with respect and dignity. 17.To participate in the induction of colleagues and contribute to changes to working practice within own speciality. 18. 0.To attend and participate in relevant/mandatory training. 19.To attend and participate in supervision and the Councils Personal Review and Development Scheme [PRADS] 20.To undertake any other work appropriate to the level and general nature of the posts duties,
