About the Role: Hospital Team
We are seeking two experienced and motivated Social Workers to join the Lambeth Hospital Social Work Team, supporting Lambeth residents admitted to King’s College Hospital (KCH), Guy’s and St Thomas’ (GSTT), and hospitals outside the borough. This is a fast-paced, collaborative environment where you’ll play a vital role in delivering strength-based, preventative support and safeguarding interventions.
Key Responsibilities:
- Hospital Discharge & Safeguarding: Facilitate safe and timely discharges, ensuring appropriate support and safeguarding measures are in place.
- Care Act Assessments & Reviews: Conduct assessments and reviews of existing care packages, including consideration of placements and reablement referrals.
- Multidisciplinary Collaboration: Work closely with hospital MDTs, voluntary sector organisations, and borough-wide teams to ensure holistic support.
- Reablement & Independence: Refer individuals to reablement services to help maximise their independence and daily living skills.
- Client Group Support: Provide support to adults (18+) with physical health disabilities and vulnerabilities.

Hybrid Working: Required on-site 3 days per week, with the option to work remotely for 2 days.
Key Requirements:
- Professional Registration: Social Work England registration and enhanced DBS check.
- Experience: Demonstrated experience working with adults in hospital or community settings, including safeguarding and complex case management.
- Knowledge: Strong understanding of the Care Act, Mental Capacity Act, and safeguarding procedures.
- Teamwork & Communication: Excellent collaboration skills with a commitment to person-centred, strengths-based practice.
- Adaptability: Ability to thrive in a dynamic, fast-paced environment and contribute to positive outcomes for residents.

About Us:
Join the Intermediate Care Lambeth (ICL) At Home Team—a collaborative, multidisciplinary service designed to provide urgent, community-based care that prevents unnecessary hospital admissions. We bring together staff from Lambeth Adult Social Care and Guy’s & St Thomas’ Hospital Trust to make a meaningful difference in the lives of Lambeth residents.
The Role:
We are looking for an experienced and compassionate Social Worker to join our dynamic team. As part of this integrated service, you will play a key role in supporting individuals in the community, ensuring their needs are met through person-centred care and collaborative teamwork.
Key Responsibilities:
- Care Act Assessments & Support Planning: Carry out thorough assessments and develop personalised care plans to support Lambeth residents.
- Home Visits & Safeguarding: Conduct urgent face-to-face visits and address safeguarding concerns.
- Carer Support: Assess the needs of carers and ensure access to the right support and services.
- Mental Capacity Assessments: Complete mental capacity assessments within the legal framework, ensuring that all individuals are supported appropriately.
- Duty Coverage & Multidisciplinary Collaboration: Work alongside health professionals to screen new referrals and participate in regular multi-disciplinary team meetings.

Rehabilitation & Reablement Services: Identify and support cases that require the Rehab & Reablement pathway, helping residents regain their independence.
Key Requirements:
- Professional Registration: Social Work England registration and enhanced DBS check.
- Experience: Proven experience in managing complex caseloads, delivering strengths-based and person-centred care.
- Communication Skills: Strong engagement with service users, their families, and colleagues. A commitment to Equality, Diversity, and Inclusion.
- Flexibility: The role is office-based five days a week when working as the triaging/duty Social Worker (approximately one week per month). However, hybrid working is available for the remaining weeks, with a commitment to two office days per week.
