Summary
An exciting opportunity has arisen for a dynamic and compassionate Clinical Nurse Specialist to join our Inclusion Health team. Based primarily on inpatient wards, with some outreach, this role focuses on improving outcomes for inclusion health patients.
You will provide expert clinical assessment, support complex discharge planning, and work closely with multidisciplinary teams and community partners to reduce health inequalities.
We are looking for an experienced nurse with excellent communication skills, a trauma-informed approach, and a passion for delivering person-centred care to those who need it most.
