The opportunity has arisen for 2 dynamic individuals to work in a new, exciting and evolving role as a Homelessness Support Practitioner as part of the Denbighshire Homeless Prevention service.
The post holders will undertake a broad and creative range of tasks working with our Social Worker, and other multidisciplinary team members, to facilitate timely move on to permanent accommodation, whilst enabling individuals and their carers to achieve definitive outcomes that matter to them to maximise potential, regain or maintain independence, with a view to sustaining a tenancy in the long term.
This is an opportunity to work in a brand new, innovative role and shape it to form an essential part of the multidisciplinary team. Working with households in a person centred way ensuring that practice is fully psychologically and trauma informed.
