Summary
1. Reviewing all non-coronial deaths within both Primary & Secondary Care. 2. Supporting and advising colleagues when discussing proposed causes of death 3. Working closely with Governance, PALS, Morbidity & Mortality 4. Liaising with H M Coroner & Bereaved relatives, where required. 5. Identifying & Escalating themes and trends. 6. Improving death certification
