Job Title: Advanced Nurse Practitioner Employer:Wandsworth Primary Care Services Ltd Location:Bolingbroke MedicalCentre, Wakehurst Road, London SW11 6BF Hours of work: Mon-Fri,37.5 hours per week Responsible to:Dr Hima Puvinathan / MissSusan Soe Accountable to:Wandsworth PCN's joint Clinical Directors Salary:Band 8a dependent on experience (£61,927 £66,384) Job Type:Permanent Job Summary The Advanced Nurse Practitioner(ANP) will be competent in using their expert clinical knowledge and skills. They will have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients. The ANP will work with their patients to achieve optimum health outcomes. They will be autonomous in making decisions based on assessment, diagnosis and interpretation of test results. They will be able to independently prescribe appropriate medication, evaluate or refer to other specialists if necessary. Primary Key Responsibilities The following are the core responsibilities of the ANP. There may be, on occasion, a requirement to carry out other tasks, dependent upon workload and staffing levels: To manage patients presenting with a range of acute/chronic medical conditions, including undifferentiated undiagnosed conditions, providing subject matter expert advice To identify red flags and underlying serious pathology and take appropriate action To use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice To implement and evaluate individual specialised treatment plans for chronic disease patients To identify, manage and support patients at risk of developing LTCs, preventing adverse effects to the patients health To provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF To assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team During 1 day/week on call duties, to provide clinical decision-making and care for on-the-day urgent cases, including telephone triage, face-to-face consultations, or home visits as necessary To actively take a personalised care approach and population centred care approach to enable shared decision making with presenting patients During 1 day/week on call duties, to respond to and manage urgent medical queries from patients or clinical staff To provide wound care (ulcer/Doppler, etc.) To provide specialist clinics such as ear syringing, contraception, etc. To undertake the collection of pathological specimens To provide travel medicine services To request pathology services as necessary To process and interpret pathology and other test results as required in a timely and clinically appropriate manner (especially during 1 day/week on call duties) To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required To develop, implement and embed health promotion and well-being programmes To maintain accurate and up-to-date clinical records in line with practice and regulatory standards as well as in conjunction with extant legislation To ensure SNOMED CT codes are used effectively To maintain chronic disease registers To develop, implement and embed well woman/well man/travel clinics and effective call/recall systems To be responsible for the implementation of an effective immunisation programme To chaperone patients where necessary To prioritise health issues and intervene appropriately To support the team in dealing with clinical emergencies To recognise, assess and refer patients presenting with mental health needs To implement vaccination programmes for adults and children To review and process clinical documents during 1 day/week on call duties, ensuring accurate workflow and appropriate coding or follow-up actions To be an extended and supplementary prescriber, adhering to extant guidance To support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required To contribute to practice targets (QOF, etc.), PCN targets and service requirements (IIF, etc.) complying with local and regional guidance To complete all relevant training in order to provide multi-professional clinical practices and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.) To delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual) During 1 day/week on call duties, to collaborate with the wider clinical team to ensure, safe, effective and efficient care delivery. To support the clinical team with all safeguarding matters in accordance with local and national policies To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately To deliver opportunistic health promotion where appropriateSecondary Responsibilities In addition to the primary responsibilities, the Advanced Nurse Practitioner may be requested to: Act as the audit lead, effectively utilising the audit cycle Act as the IPC lead for the practice, ensuring compliance, undertaking audit as necessary Monitor and ensure the safe storage, rotation and disposal of medicaments Undertake overall management of the nursing team, providing guidance when necessary, acting as a mentor to students and newly qualified staff Maintain an effective nursing staff rota ensuring all clinics are staffed appropriately Participate in local initiatives to enhance service delivery and patient care Support and participate in shared learning within the organisation Develop an area of specialist interest, taking the lead within the organisation Continually review clinical practices, responding to national policies and initiatives where appropriate Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA) Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as required Develop practice administrative and clinical protocols in line with the needs of the patient and current legislationGeneric Responsibilities Confidentiality In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters.
