Summary
Main duties of the job Clinical Leadership Provide effective clinical leadership to the nursing team. Champion evidence-based nursing care and best practices. Lead on clinical governance, quality assurance, and safety initiatives. Patient Care & Chronic Disease Management Oversee chronic disease clinics and long-term condition management. Deliver high-quality care across treatment planning, implementation, and review. Promote proactive health education and disease prevention strategies. Deliver own nurse-led clinics (e.g., asthma, diabetes, hypertension). Team Management & Mentoring Supervise, mentor, and appraise nursing staff and students. Support professional development and performance improvement. Manage nursing rotas to ensure effective clinic coverage and staff balance. Policy, Compliance & Clinical Standards Develop and review nursing protocols and clinical procedures. Ensure compliance with CQC regulations and NHS standards. Participate in audits, QI initiatives, and root cause analyses for incidents. Collaboration & Communication Work closely with GPs, the Practice Manager, and multidisciplinary colleagues. Liaise with external agencies, community services, and safeguarding teams. Represent nursing in practice meetings and strategic planning. Core Clinical Duties Immunisations: Routine, childhood, travel, and seasonal Cervical cytology and womens health services. - Chronic disease management including diabetes, asthma, COPD Wound care, suture removal, ECGs, Venepuncture, NHS Health Checks and opportunistic health promotion Safeguarding children and adults; Mental health assessments Requesting/interpreting pathology results Referral coordination Infection prevention and control (IPC) Support with audit cycles and service evaluation Ensure proper use of SNOMED CT coding and chronic disease registers Uphold best practice in medication management and PGDs Encourage team learning and contribute to the practice's continuous improvement
