Summary

CLINICAL RESPONSIBILITIES 1.To demonstrate specialist clinical expertise in the assessment, differential diagnosis, treatment and management of swallow and communication problems in adults with acquired disorders, including those with tracheostomies and who are ventilator dependant, providing appropriate intervention and evaluating outcomes. 2.To demonstrate specialist knowledge in this clinical area underpinned by current evidence based practice and outcome measures. 3.To demonstrate highly developed auditory and perceptual skills in the assessment, diagnosis and treatment of patients. 4.To make appropriate differential diagnoses and specialist clinical decisions based on evidence from assessment of complex cases, seeking advice as appropriate. 5.To develop knowledge and skills within each specialty, consulting with clinical supervisor as appropriate. 6.To apply specialist knowledge to inform sound clinical judgements/decision making for case management, developing clear care plans based on best practice. 7.To provide advice to others regarding the management and care of patients with communication and/or feeding and swallowing difficulties. RESPONSIBILITY FOR PATIENTS 1.To develop a working knowledge of relevant procedures including: E.g., Working with Vulnerable Adults and other legal frameworks. 2.To communicate complex condition related information from assessment to patients, patients, carers, families and members of the multi-disciplinary team/other professions, demonstrating empathy and ensuring that effective communication is achieved, particularly where barriers to understanding exist. 3.To develop skills in motivating patients and /or carers to engage in the therapeutic process 4.To employ counselling skills with patients and carers. 5.To work closely with patients, carers and families, in order to agree plan of management. RESPONSIBILITY FOR POLICY AND SERVICE DEVELOPMENT 1.To comment on proposed service/policy developments using specialist knowledge to inform these as appropriate. 2.To collaborate in the development of local clinical guidelines, informed by current specialist clinical evidence. 3.To work collaboratively with others to the develop projects and therapy packages. 4.To participate in the Personal Development Plan and CPD process, ensuring that the objectives set reflect the plans of the Trust and the Speech and language therapy service including specific Service Development Review (SDR) objectives relating to each specialist area when in that part of the rotation. **For further information please refer to the attached JD/PS**
