Summary

Provides expert clinical knowledge to Nursing and Junior Medical Staff and acts as a clinical role model, demonstrating skills in identifying patients at risk and initiating treatment to critically ill patients regardless of location Perform autonomous comprehensive assessments of critically ill patients, plan, implement and evaluate care delivery according to changing health needs and varying levels of complexity Independently identifying patients at risk, both prior to and following discharge from Critical Care Maintenance of track and trigger score and escalation when required Active member of ward based MDT working to rehabilitate Critical Care step down patients, identify psychological problems, offering informal support and where appropriate referring on for formal investigation or follow up by the relevant specialty Coordinate and maintain effective communications between Critical Care and other ward/departments, managing conflicting views where necessary Works collaboratively with other specialties to provide specialist care for patients recently stepped down from Critical Care, those at risk of acute deterioration and those requiring specialist support for example, tracheostomy and laryngectomy patients in cohort wards and Clatterbridge. Initiates prescribed treatment to the acutely ill ward patient during an emergency and ensures full management plan delivered and evaluated Coordinates the inter hospital transfer of acutely ill level 1,2 and 3 patients for investigations or to an area for a higher level of monitoring Leads in the delivery of education, training and development of staff throughout the general/cohort wards on tracheostomy and laryngectomy care
