Summary

The post holder will work as a first point of contact within a fully integrated and multi-disciplinary team in the delivery of meaningful, person centred and solution focused interventions which focus on the person as the expert in their life. They will be skilled in using active listening and solution focused skills on a daily basis to enable people to shape their own lives, identify their own goals and have the confidence to sustain independence. This will mean facilitating knowledgeable advice and information, and if appropriate, access to further assessment and service provision.
The post holder will seek to help the person identify individual need and where appropriate assist with support planning and brokerage using the online and community market place to ensure people get the right care and support at the right time.
The post holder will offer a flexible, immediate response to people, their families and carers. They will refer, liaise, consult and work effectively with a range of professionals, partner agencies and networks in order to build strong effective partnerships which support integrated and multi-disciplinary working.
You should have a good standard of general education, including good applied literacy and numeracy skills.
