Summary

Using clinical expertise in applying the Continuing Health Care Framework for Children, the post holder will be responsible for Assessing and Reviewing individuals within or referred into CYPCC. The post holder will work with wider teams to support the delivery of individualised care delivery including Personal Health Budgets (PHBs). Working within the remit of the National Framework (NF) for CYPCC, the post holder will provide a high quality service that supports the delivery of an effective and competent service that consistently delivers an individual-focused intervention with patients, their families and representatives. Take a lead on ensuring transition processes are continually developed and improved to achieve seamless CYP key transition milestones. The post holder will be required to have a good knowledge of health and social care practice and be able to demonstrate an understanding of CYPCC and be prepared to extend their skills, knowledge and competence in this field. This post will require good communication skills with individuals and their families or representatives and requires the ability to work in a multi-disciplinary way with colleagues. Deliver against objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines. The post holder will be responsible for ensuring their professional registration and mandatory training is up-to-date.
