Summary

Key Responsibilities: Provide high-quality clinical assessment, diagnosis, and management of patients presenting with minor and potentially serious acute conditions in general practice and care home settings. Work alongside duty GPs and other ACPs as part of the duty team to deliver timely, evidence-based care. Conduct assessments via telephone triage and face-to-face appointments, prioritising patient needs effectively. Take comprehensive patient histories, perform physical examinations, and identify red flag symptoms. Develop differential diagnoses and formulate appropriate management plans, including arranging investigations and analysing results. Prescribe and review medications in line with national and practice protocols, ensuring therapeutic effectiveness. Support patients and carers with education on health conditions, promoting self-care and informed decision-making. Carry out palliative care planning, including anticipatory care, care home reviews, and completing ReSPECT forms and care plans. Provide ongoing clinical reviews and support in care home environments, ensuring continuity of care. Collaborate with the wider healthcare team to ensure seamless patient care and appropriate referrals. Maintain accurate, complete, and up-to-date patient records in accordance with legal and practice standards. Participate in team meetings, contribute to quality improvement initiatives, and support a safe working environment. Communication & Service Delivery: Demonstrate sensitive and effective communication tailored to individual patient needs, cultural backgrounds, and levels of understanding. Ensure patients and carers are fully informed and consent to treatments. Recognise and overcome communication barriers to improve patient engagement. Contribute to a high standard of service delivery by working within your scope of practice and adhering to policies and procedures. Engage in continuous professional development and performance reviews to maintain and enhance skills. Teamwork & Professional Responsibilities: Understand your role within the team and support colleagues to improve patient care. Delegate tasks appropriately and support the development of new ways of working. Promote a culture of safety, quality, and continuous improvement. Be aware of legal responsibilities, especially regarding vulnerable adults and children, and follow statutory procedures. Other Responsibilities: Uphold health and safety standards, confidentiality, and data protection policies.
