Summary
To providing clinical leadership to the Triage nurses and Administration staff within the Local Access Point/ Care Coordination Centre. Supporting the team to ensure safe and effective care for service users. Working alongside the Operational Manager to support the day to day running of the Team and development of staff within the triage team. To provide a comprehensive triage assessment to service users involving complex decision making to ensure that service users access the appropriate service in a timely manner. To be experienced in a range of nursing procedures including but not limited to wound care; end of life care; continence management; administration of medications through various routes including I.Vs. To act as a role model and a senior member of the Local Access Point sharing clinical knowledge, expertise and advice and provide mentorship and supervision to pre and post-registration nurses. To ensuring robust escalation and business continuity plans are in place and delivered during periods of high demand and/or reduced capacity. To working closely with other MPFT services and external partners to support the development and delivery of efficient and effective care pathways. Supporting the delivery of place based care within the attached locality. To understands population health needs within attached locality and develops collaborative working and maintains networks within the community, acting as a key stakeholder to ensure these needs are met. To contribute to service/policy development through active membership on working parties or professional forums. To be aware of national and local policy that impacts upon the health and wellbeing of service users. To review incidents and share any learning identified.
