Summary
erform specialist personalised assessment and higher level discussion of patients holistic needs Demonstrate higher level clinical care by managing a patient caseload providing expert assessment, planning and evaluation Contribute towards a multi-professional approach in the management of patients with the specialist area, especially in relation to on- going care needs and discharge arrangements, including access to community cancer support Establish and maintain effective communication with patients and carers/relatives Support patients and their families and ensure that they receive the required information to enable them to self- manage, lead or participate in their care delivery. Recognise changes in patients condition that requires the intervention of others and case manage/refer on as appropriate Support, assist and encourage the multi-disciplinary team in resolving patients problems Deliver high standards of patient care Work in partnership with Lead Cancer Nurse, clinicians and wider team to ensure delivery of high standards of patient care Work collaboratively with other team members to promote development and provide training to other nurses A full job description is attached
