Summary


ThepostholderwillbepartoftheHomeFirstServiceDeliverythiswillrequiretoworkacross and develop skills in all areas within the pathway, these include:

· RapidResponseservice

· UnplannedPhysiotherapy

· Community Rehabilitation Team (Formally Reablement)

· Specialist Fallsservice

· Hospice

· VirtualWard

· IntermediateCareWard

· EnhancedCareHomeTeam



To contribute to the delivery of a 7-day working service. Although the role will be predominantly be within the Enhanced Care Home Team and Community Sector, there may be an expectation to complete assessments, support team or work on inpatient rehabilitation wards as required.

• To provide specialist physiotherapy assessments over a range of conditions and complexities using advancedclinicalreasoningtoanalyseclinicalandnon-clinicalfindingstoformaccuratephysiotherapy diagnoses and prognoses.



• ToassesspatientunderstandingofassessmentandtreatmentinaccordancewiththeConsentPolicy, andtoassistmembersoftheteamtoworkwithinthelegalframeworkforpatients wholackcapacityto consent to treatment.



• Toprovidehealtheducation,healthpromotionandpreventionadvice.



• Tomanageaclinicalcaseload,organisingthiseffectivelyandefficientlywithregardtoclinicalpriorities and use of time with access to expert support as required, both for self and more junior staff, withinMDT.



• Toformulateandrecommendtoothersthebestcourseofphysiotherapyinterventionand/ortreatment programmes to meet the agreed goals set with the individual patient/or carer, using evidence based practice and a wide range of treatment skills and options.



• Toevaluatepatientprogress,reviewgoalsettingandaltertreatmentprogrammesasrequired.



• Tousearangeofverbalandnon-verbalcommunicationmethodstocommunicateeffectivelywith patients and carers to maximise rehabilitation potential.
