Summary
Clinical: To be the designated RN for Le Murier and a small cohort of students with complex health needs who attend the College of Further Education. To ensure each child has a comprehensive assessment and care plan, outlining health needs, prior to the child commencing school, taking into account the individual psychological, social, physical, and spiritual needs of the child or young person. To assess, plan, implement, and evaluate the nursing needs of the children during the school day, working in partnership with education, CYP, families and the multi-agency teams to meet health, holistic needs of the individual. To act as a link between health, education, and other members of the multidisciplinary team, to ensure that the health needs of the CYP on the caseload are being effectively managed, enabling access to education, within special needs school settings. To provide nursing advice and support to parents, carers, school staff and other health care professionals and to take an active role in multi-agency planning. To ensure effective medicine management for children and young people at the school. Participate in medicine audits and have a clear understanding of the medicine management policy and procedures within a school environment. To perform clinical nursing procedures as required and to provide support to educational staff to enable them to respond appropriately to emergency situations. To take an active role in Community Paediatrician health clinics within school and with medicals, hearing, visual and any other health checks and services that are required. To facilitate early recognition of health, social and emotional problems. To work closely with colleagues within children and adolescent mental health services (CAMHS). To undertake evidence-based child health assessments and develop child health plans in partnership with children, young people, parents, and carers. To embrace the public health role, promoting the heath of students and the wider community, through school and community-based health education programmes and activities. To promote positive health education as an integrated aspect of care delivery, preventing ill health where possible. To support the effective transition of students between schools and into adult services, acting as an advocate for the student. To support the immunisation programmes, providing a bespoke service that meets the individual need of all students, ensuring optimum uptake. To participate in multi-disciplinary meetings acting as Lead Professional as appropriate. To identify children in need and children in need of protection and work with other agencies to support vulnerable families. This should include providing reports for and attendance at child protection conferences, core groups, and child in need meetings. To work in line with Islands Safeguarding Children Partnership guidelines. To participate in regular discussion with managers about children giving cause for concern. To support children in care in achieving their optimum health by undertaking regular health reviews as per policy and giving individualised health care and advice as required, liaising with the CIC nurse as needed. Partnership with Service Users: To undertake parental consultation and perform risk assessments for children starting school. To provide information and support to facilitate the childs and familys own choices with regards to nursing care and to produce care plans with appropriate professionals, children, and their parents. To support efficiency and pathways between home, health, and social care agencies as appropriate to enable readily accessible information for each childs health needs within school. Leadership/Managerial: To develop guidelines and protocols to support non-clinical school staff to provide a safe and evidence-based service to all students. To work alongside the teacher responsible for Child Protection / Safeguarding to be aware of concerns and act according to school and health policy, and to attend meetings as appropriate. To support the implementation of service development, standard setting, clinical audit and benchmarking, to deliver needs-led evidence-based care that supports the local and national priorities and policies. To undertake regular meetings with the school senior leadership team, to ensure staff are kept updated and informed on all relevant matters pertaining to the workload, particularly of any incidents or matters requiring managerial / professional advice, and to assist within the system of recording shared risks. To take every reasonable step to see that the working environment is safe, healthy and hazard free for staff and users, including ensuring that infection control procedures are in place. To support the Head Teacher with the management of the health/medical budget for the school, ensure the appropriate procurement for equipment and consumables. To be a source of clinical expertise around learning disabilities and associated conditions, including mental health and wellbeing. To monitor, control and store equipment/resources according to the requirements/specifications of the clinical environment. To ensure the safe and efficient use of equipment, resources, and consumables. Training and Education: To identify, alongside the Head Teacher, the training requirements for the wider school staff group and assist with monitoring compliance for mandatory training, including but not limited to the use of EpiPens, oxygen therapy and seizure response. To facilitate training and implement teaching plans for parents, school staff to empower them to manage a child/young persons condition. To play an active role in the in-service training programme by attending, contributing to and presenting at staff meetings, tutorials, training sessions, external courses and engaging in reflective practice. To participate in clinical and safeguarding supervision, annual performance appraisal as recommended in the Health and Social Care (HSC) policies. To undertake annual statutory, mandatory, and essential to role training as identified by the Department. To identify own training needs and access training as agreed with line manager. Communication: To co-ordinate complex care arrangements through communication with multidisciplinary team members and associated agencies. To utilise various communication strategies and systems to support effective communication with students and parents, including but not exclusive to those with a hearing loss, visual impairment, social and emotional communication difficulties, English as a second language. This may include play therapy, sign language or the use of interpreter services. To support the student and their carer to understand their condition, using a wide variety of communication tools, providing sensitive and emotional information around the diagnosis and/or outcomes, which may be life limiting. Quality Assurance: To maintain accurate, contemporaneous professional records in accordance with NMC guidelines and HSC policy. To ensure that there are Clinical Governance mechanisms and effective monitoring systems in place to assure the quality of service. To work closely with other members of the School Nursing Team and multi-disciplinary special schools team to ensure equity to service delivery and best practice. To participate in clinical audit, as required within the department. To participate in regular clinical and safeguarding supervision with identified health professionals.
