UNLOCK YOUR BEST WORK LIFE + MAKE A DIFFERENCE TO EVERY LIFE We have an exciting opportunity for a Social Prescribing Co-ordinator to join our healthcare teams across our South Yorkshire team, working across HMP YOI Doncaster, HMP Moorland, HMP Hatfield and HMP Lindholme. As a Social Prescribing co-ordinator you will be responsible for connecting people to activities and services within the prison, and required to support and motivate patients to access opportunities to help improve their health and wellbeing, whilst reducing dependency on medication. This is a new initiative being brought in across our Yorkshire Region. All work will be based on site, you will be expected to work at all sites as part of this role. Hours 9am-5pm Monday to Friday Salary This position is full time, youll receive an annual salary up to £33,000 per annum - depending on experience. How will we support you?
