Your Role:
As a key member of our team, you'll provide high-quality, proactive care tailored to the needs of your designated neighbourhood population. Utilizing your specialist skills in assessment, planning, and implementation, you'll deliver person-centred care that makes a positive impact.
Key Responsibilities:
• Collaborate with Primary and Secondary care clinicians, hospital teams, voluntary organisations, and other professionals
• Ensure the right care is delivered in the right place, by the right professionals
• Share caseloads within the multidisciplinary team to maximise efficiency and patient outcomes
• Demonstrate leadership, teamwork, delegation, and excellent care planning
• Manage medication, health, and case coordination for individuals with complex needs
Ideal Candidate:
• Proven expertise in care planning, assessment, and coordination
• Strong leadership and teamworking skills
• Ability to work collaboratively within a multidisciplinary team
