The Integrated Disability Service for children and young adults up to the age of 25 in Conwy are looking for a Social Worker to join our team.
With a focus on what matters to the person, helping children and young people aquire, re-aquire, maintain or develop their potential, the Social Worker will work to provide a service that is person centred and outcome focussed.
You will have the opportunity to work in partnership with Health and Education, the Social Worker will implement an integrated approach to meeting the social, educational and health needs of children and young adults.
We are looking for a Social Worker with vision and passion, who are able to develop effective relationships and shared responsibility for supporting children and young adults with Disabilities to live the lives that they choose.
