Summary

Promote Independence: Help individuals live independently and prevent hospital admissions.
Facilitate Hospital Discharge: Ensure smooth transitions from hospital to home.
Maximize Community Living: Support independent living in the community.
Assess Care Needs: Work closely with District Nursing, Social Care, and Housing Support colleagues to provide person-centered care.
Role: Stepdown Support Worker

As a Stepdown Support Worker, you will:

Support Independence: Assist service users in living as independently as possible.
Provide Personal Care: Offer basic health care and therapy, including mobility practice and exercise supervision.
Collaborate with Professionals: Work with District Nursing, Social Care, and Housing colleagues to deliver comprehensive care.

We are a friendly and supportive team, including:

Registered Manager
Care Manager
Shift Leader
Support Workers
You will be well-supported and work under the direction of a shift leader or care manager, helping service users achieve their re-ablement goals.

Position Details

Schedule: Includes weekends and bank holidays on a rostered basis, with split shifts between 7am and 10pm.
