Summary


To be a core member of the multi-disciplinary team, participating in the provision high quality mental

health care for people who present with severe and enduring mental health problem, with complex

needs.

Successful candidates will work in multi-disciplinary recovery based community mental health

services providing evidence-based care and treatment for all those who access the service through

the duty system. You will provide treatment within structured Care Pathways that ensure the best

outcomes for our people who use services, with a focus on recovery and promoting independence.

You need to have experience of working with people with severe and enduring mental health

problems as part of your training and/or preregistration experience and have a commitment and

genuine interest in supporting service users and carers through Recovery model in the community.

This role will include working with an allocated caseload of new and existing service users and using

a range of clinical, psychological and social care interventions to enable service users and carers to

meet their recovery aims and maintain positive outcomes for both their health and wellbeing.

Take and respond to calls that come through the team duty system ·

To proactively involve people in their care, and to include them in decision-making processes. ·

To take responsibility to act using sound professional judgment and effective problem solving when decisions, with the support of your line manager / supervisor, when managing in complex and challenging

Act as a care co-ordinator responsible for managing a caseload of service users in the community, and ensure the provision of high quality and evidence based mental health care.

Assess, plan, implement and review care in accordance with CPA and trust policies.

Will conduct comprehensive mental health assessments and to develop coproduced care and support plans with service users and carers accordingly
