Sunderland Care and Support, work closely with Adult Social Care to support professionals and customers who contact Adult Social Care as they require advice or support regarding appropriate solutions to their social care and support needs
We are looking for a skilled and motivated workers to undertake the role of a Care Navigator the purpose of this role is:
To manage the ‘first conversation’ of a contact assigned to the ‘Front Door’ – taking action to ensure the right support is provided to maintain or improve the well-being of the person
To undertake an initial triage to determine the requirements of a person to ensure the person receives the right type and level of care and support to meet the persons needs
To act as a point of contact to Health and Social Care professionals and general public
To liaise with a range of providers in order to highlight potential support or interventions that they may be able to offer that could meet the persons needs
You will collect and record information using a variety of computer systems, you will liaise with health and social care professionals via the telephone as well as via email and face to face contact.
You will help to support both the community as well as the hospital in progressing referrals to the appropriate services, helping to prevent unnecessary admission to hospital ensuring the customer receives the right level of care and support to meet their needs, you will also support services during extreme pressure helping achieve a positive outcome for the customer.
We are looking for a suitably skilled individual, who has experience in this field of office-based work. You should have excellent written and communication skills and good IT and typing skills as well as having a good understanding, issues and situations including safeguarding.
The role will include working a Monday to Friday rota, excluding evenings, weekends and Bank Holidays.

This post is a 18.5 or 37 hour Grade 4, Salary £27,711- £30,060 (pro rata if working less than 37 hours)
