Summary


The aim of the post is to support the specialist palliative care team to maximise the quality of life of patients with advanced life limiting disease and their families/carers, facilitating their care throughout their disease trajectory and into bereavement, and where possible, helping patients die in their place of choice.

The post holder will work alongside senior specialist palliative care practitioners in supporting and assessing patients and develop their own clinical practice through independent, supported practice.

The post holder will provide specialist palliative care advice and support to people affected by life limiting illnesses. This will be achieved both directly through clinical practice and caseload management and indirectly through learning, education, research, audit and professional activities taking into account corporate and directorate objectives and the clinical governance framework.
