Summary
The role is specific to supporting those who aim to improve health outcomes, especially for those who continually consume alcohol, where traditional structured treatment will not be appropriate.
The aim of the service is to help those with a range of health needs, including drug and alcohol use, access appropriate treatment and address clients' physical and mental health. The service will provide essential health promotion to increase the number of clients engaging with services to address their health and well-being.
• Patient Assessment:Conduct initial assessments to evaluate addiction severity and monitor patient progress.
• Treatment Planning:Collaborate with an interdisciplinary team to develop and implement individualised treatment plans.
• Medication management -issuing scripts for patients on OST andpatients on alcohol community detox.
