More detail about the role The Practice Educator will work with community multidisciplinary teams to support team members in the management of patients who are referred to the urgent community response team for admission avoidance or to enable timely discharge from an acute/community setting, ensuring that therapy input is integrated into the patients overall care plan. The Practice Educator will work as an autonomous practitioner, working without direct supervision and at times lone working. The post holder will provide supervision, training and support for community teams by; Supporting unregistered clinical staff to undertake clinical activities, and support the delivery of accredited and nonaccredited education with particular focus on essential and required learning for the different roles to ensure competencies are reached and maintained. Supporting registered clinical staff in undertaking assessments and mentoring unregistered staff with their clinical skills development, ensuring safe and fair assessment of learners. Supporting in the delivery of structured classroom-based training where required, this includes; Medicines Administration, UCR Rehabilitation Programme and competencies, recognising a deteriorating patient and Manual Handling updates. Work with managers to develop the service in line with patient need and trust wide developments, and to help provide an equitable service across the Trust. Working Pattern: Majority Monday-Friday within 8am-6pm boundaries. Occasionally staff are called upon to cover 8am-8pm working, weekends and Bank Holidays Interview Date: To Be Confirmed Pay: The salary advertised for this position reflects the Agenda for Change 2025/6 rates, inclusive of a 3.6% pay award which will be implemented later this year. If you join before the implementation date, the pay award will be backdated to your start date.
