Summary

With more emphasis on care being delivered in the community to prevent hospital admissions and facilitate early discharge, there has never been a better time to join the Community Children’s Nursing Team in Tower Hamlets.

We are looking for a motivated individual with excellent communication, interpersonal and organisational skills to join our team, to deliver care within an integrated way across the community. This is a 24 month fixed term contract to cover study leave.

Your focus will be on delivering day to day evidence-based nursing care and case coordination for children and young people aged 0-18 with acute, short- and long-term nursing needs. You will adopt a person-centred care approach, focusing on the individual needs of the child and their families, encouraging active engagement to empower and create independence whenever possible.

To achieve the best health and well-being outcomes for every child referred to the Community Children’s Nursing Team (CCNT).

To facilitate early discharge of children from hospital and reduce or prevent hospital
re-admission.

To work together with parents to produce a joint plan of care to meet the needs of their child.
To enable and empower parents to look after their child with our support.
