The post holder will be a member of the Immunisation Team whose role is to deliver
immunisations as part of the Department of Health (DH) programme with particular
emphasis on delivering the school age programme of vaccinations
Have responsibility for the assessment, planning, delivery, and evaluation of evidence based quality care to an allocated group of patients on a shift basis.
Work in partnership with patients and those that matter to them, involving them in decisions
about their care.
Recognise any deterioration or change in a patient’s condition which may require the
intervention of others and refer as appropriate.
Work collaboratively as part of the multidisciplinary team, establishing and maintaining
effective communication and relationships.
Provide early intervention to ensure the safe discharge of patients, working in collaboration
with other relevant teams and services to prevent unnecessary readmission of patients.
Act as an advocate for the patient and their family, protecting their interests and statutory
rights, whilst promoting privacy, equality, diversity, and inclusion.
Maintain a professional image in accordance with the uniform policy and uphold the Trust
values and behaviours, promoting One Culture of Care.
Direct and supervise the work of non-registered colleagues including, but not limited to,
healthcare support workers and student nurses.
Ensure essential safety training and role specific training is completed in line with Trust
policy.
Take responsibility for own continuing professional development and performance,
We employ more than 6,500 staff who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, health centres and in patients’ homes. We also are incredibly proud to have almost 150 volunteers here at CHFT.
We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children’s and young people’s services; end of life care and outpatient and diagnostic imaging services.
We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.

Plan and organise immunisation sessions in schools and where necessary, alternative community
settings and clients’ homes.
Deliver the school age immunisation programme in line with current medicines management policy
and procedures.
Provide advice, guidance and support to schools to achieve the successful delivery of the schoolbased immunisation programme
Promote the DH Immunisation Programme by supporting and providing effective health promotion
activities to both large groups and on an individual basis.
Immunise school aged and when required, pre-school and adult populations.
Work within Calderdale Safeguarding Children’s Board policies and procedures.
Promote the positive health of school age children by supporting the provision of quality health
promotion activities.
Work with parents/carers to achieve optimum immunisation uptake.
Advise and co-ordinate healthcare to children with complex medical/special needs specific to the
immunisation programme.
Target and support vulnerable groups to achieve optimum immunisation uptake.
Support and empower children, young people, and their families to make healthy lifestyle choices
in respect of immunisations.
Ensure that appropriate care is provided for children and young people pre, during and post
immunisation, including emergencies.
Provide confidential support to children young people and their families regarding the immunisation
programme and associated conditions.
Offer supportive and confidential services to promote positive health outcomes in respect of
