What You'll Do:
• Provide initial assessment and support to new and known service users.
• Offer expert guidance and information to healthcare professionals.
• Conduct urgent assessments for individuals needing immediate help.
• Work collaboratively within a multi-disciplinary team.

In this role, you will play a vital part in providingholistic, patient-centered care. You'll conduct comprehensive assessments, considering all aspects of a patient's well-being - mental, physical, social, cultural, and more. By working collaboratively with patients, their families, and other healthcare professionals, you'll develop personalized care plans that address their unique needs.
